The demographic information will include your address, phone, insurance information and will be securely submitted to our electronic medical records. Then download and fill out the History documents and bring them to your appointment! LAP-BAND patients have an additional form to complete.
Patient Confidentiality and HIPAA
The Health Insurance Portability and Accountability Act (HIPAA) was passed by Congress in 1996. Its primary purpose is to improve the efficiency and effectiveness of the health care system, help providers access patients’ health care information, standardize the way the information is handled, and ensure that patient health information remains strictly confidential.
No issue is more sensitive for people than keeping their personal health information private. Patient confidentiality is a top priority for us. The HIPAA law was enacted to provide safeguards for personal health information from disclosure, fraud, and abuse. Health care providers are required by law to notify all patients of the health care provider’s privacy practices. By law, no health care can be provided until you acknowledge the receipt of our privacy practices. You can download a copy of our privacy practices in the Download section below. We apologize for the inconvenience of this additional paperwork in an already complicated health care system.

You may need to download Adobe Acrobat Reader to view the documents below. Fill out your Registration and the Health History by clicking each of the links to place your information into our system. The Health History link will generate an email that you should immediately send. Filling out these sections will eliminate a lot of the usual paperwork when you come into the office which will make your visit more efficient.
If you are unable to send these forms you can print them and fax them to us or bring them to our office.
This information will be placed in your electronic medical record to significantly lessen paperwork on your visit. If you can not complete these online you can print these and bring them with you.
